1. If I had a life threatening illness today like a stroke or a heart attack, would my family know what my wishes were?
2. If I could not speak for myself would the doctor be aware of who is legally able to speak on my behalf?
3. If my memory was impaired would I be able to make these decisions for myself? If not, who would?
